My only guess is that he drove on a flat for long enough that he killed the tire, and then put it back on the spare mount because he couldn’t fit in in the back for some reason. Honestly, my first thought was, “Is that some fucked-up photoshopping?” I cannot imagine *why* someone would do that to a tire, though.
